I, the undersigned, acknowledge and agree that LETS WAKE WATERSPORTS RENTAL L.L.C. shall not be held liable for any incidents that may result in injuries, death, or other damages to me or my dependents arising from my participation in any boat tours organized by LETS WAKE WATERSPORTS RENTAL L.L.C. I declare that my qualifications in wakeboarding, water skiing, or wakesurfing are as stated by me.

I also confirm that I have reached the legal age required to sign this declaration and understand that I participate in these activities at my own risk. By this declaration, I agree to release LETS WAKE WATERSPORTS RENTAL L.L.C. from any liability for any losses, personal injuries, property damage, or death caused by negligence arising from my participation in any boating activities organized by LETS WAKE WATERSPORTS RENTAL L.L.C.

I am competent to make this declaration, and the above statements are true to the best of my knowledge, information, and belief.

At the time of signing this declaration, I am of sound mind and not under the influence of drugs or alcohol.

During the trip, all guests are required to wear life jackets. All guests must carry valid identification documents.
